Triamcinolone irrigation improves postoperative outcomes in people with chronic rhinosinusitis

In patients operated for chronic rhinosinusitis, the use of Triamcinolone irrigation is associated with reduction in crusting, edema, scarring, and POSE scores.

According to a study published in the "Ear, Nose & Throat Journal," Triamcinolone irrigation following bilateral endoscopic sinus surgery is an efficient treatment that yields excellent outcomes. The healing process is improved, and the amount of edema and polypoidal mucosal alteration in the sinus cavities is significantly reduced. Among 58 volunteers having bilateral pansinusitis, the goal of this matched-pair randomized controlled trial was to determine the impact of intra-operative Triamcinolone irrigation on postsurgery outcomes.

Each patient was randomly assigned to undergo Triamcinolone irrigation (4 ml of 40 mg/ml solution) once, followed by Merocel (polyvinyl acetal) packing on one side, and Merocel packed with saline on the opposite side, following the completion of endoscopic sinus surgery. In weeks 1, 3, 8, and 12 following surgery, nasal cavities were examined using a Hopkins rod endoscope to check for the presence of postoperative crusting, discharge, polypoidal alterations, and edema. Endoscopic staging was done using perioperative sinus endoscopic (POSE) scores.

Ages 31 to 45 years were the most frequently afflicted group, accounting for 39.7% of all cases. Males experienced more of an impact than females (63.7% vs. 36.3%). At all follow-up stages, there was a decrease in the average POSE scores at the treatment site (Table 1), along with a notable decline in scarring, edema, and crusting.

In chronic rhinosinusitis patients, Triamcinolone irrigation thus remarkably improved surgical outcomes and reduced early postoperative complications.
