To reduce one of the major side effect of propofol in morbidly obese patients, cardiovascular depression, dosing of propofol based on total body weight (TBW) should be considered.

As per the outcomes of the first-ever done analysis, the induction of propofol in people suffered from morbid obesity is associated with the significant adverse event and hence done with proper attention.

Scientists used four hemodynamics indexes comprising cardiac output (CO), systolic arterial blood pressure (SAP), mean arterial blood pressure (MAP), and diastolic arterial blood pressure (DAP) to assess the pharmacodynamic (PD) effect (cardiovascular depression) of propofol. Dosing of propofol was determined based on lean body weight (LBW) and total body weight (TBW) and the groups were divided with reference to this criterion.

As compared to control group, the patients of TBW group exhibited considerably lower CO and blood pressure at one minute of anesthesia induction. On the contrary, LBW and control groups showed no difference in their PD profiles, stating raised drug potency in MO patients. Also, a significant decrease was seen in the value of propofol EC50, however, no change was observed on other PD parameters. All these outcomes, therefore indicate that propofol is related to lower cardiovascular depression with normal weight as compared to obese weight.
