Modified Broström procedure along with arthroscopic debridement significantly alleviates pain and eliminates instability in patients with medial gutter osteoarthritis.

According to the researchers of Chungbuk National University Korea, medial gutter osteoarthritis with chronic ankle instability can be effectively managed with the help of the modified Broström procedure combined with arthroscopic debridement. The information about the outcomes of operative treatment for ankle instability with concurrent arthritic changes in the medial gutter is inadequate. The study estimated the intermediate-term radiological and clinical findings of modified Broström procedure and arthroscopic debridement among twenty-two patients with medial gutter osteoarthritis with concurrent chronic ankle instability.

The study population was followed up for more than three-years after the modified Broström procedure and arthroscopic debridement. At the time of surgery, all patients presented with the medial joint space narrowing of Takakura stage II. the visual analog scale (VAS) for medial ankle pain during walking, Foot and Ankle Ability Measure (FAAM) and American Orthopaedic Foot & Ankle Society (AOFAS) score were measured during the clinical evaluation.

Mean FAAM, pain-VAS and AOFAS scores remarkably improved from 45.7, 6.8 and 51.2  to 78.4, 3.5 and 80.3 points at final follow-up, respectively (P < 0.001). Eight patients found with gait discomfort with 4 or more pain points. The rate of recurrent ankle instability was only 4.5%; however, six patients presented with the progression of arthritis stage. In spite of arthritis onset, most of the patients involved in the study were capable of achieving the meaningful reduction in pain and reducing instability.
