In females with endometriosis, decrease of dysmenorrhea following suppressive hormonal therapy is linked with deep infiltrating endometriosis phenotype and is a marker of higher severity.

According to the findings of a retrospective observational cohort study, asking patients if their pain symptoms have subsided following suppressive hormone medication may assist practitioners in addressing the issue of the severity of  deep infiltrating endometriosis (DIE) in daily practice. The goal of the study was to determine whether a reduction in dysmenorrhea following suppressive hormonal therapy is a sign of endometriosis phenotype and higher disease severity.

Participants in this study were non-pregnant, under 42 years old, tested for relief from dysmenorrhea following suppressive hormonal therapy prior to surgery, and had histological confirmation of endometriosis. According to the findings of the suppressive hormonal test, comparisons were made.

After receiving suppressive hormone therapy, the rate of dysmenorrhea reduction among the 578 patients with histologically confirmed endometriosis who had preoperative pain sensations assessment was 87.8% (n = 510). In these subjects, an elevated rate of DIE intestinal lesions (45.7% [233/510] vs. 29.4% [20/68]) as well as the raised rate of multiple DIE lesions (≥2) (55.8% [287/394] vs. 31.7% [22/39]) were witnessed. Reduced dysmenorrhea following suppressive hormonal therapy remained substantially linked with the severe DIE phenotype following multivariate analysis (adjusted odds ratio = 3.9).
