Clinicians may use gastric
per-oral endoscopic myotomy (G-POEM) to treat gastroparesis patients as it is
safe and feasible.
As per findings of a meta-analysis, it was deduced that for treating gastroparesis with various etiologies, G-POEM is safe and feasible, and could be a promising first-line treatment choice for certain patients.
A meta-analysis was conducted to explore the safety and feasibility of G-POEM for treating gastroparesis. Through exploring databases such as PubMed, Cochrane Library, EMBASE, and Web of Science before April 1, 2019, the relevant publications were determined.
The analysis included the studies showing the clinical data of G-POEM for managing gastroparesis. The data about safety and effectiveness were extracted, pooled, and assessed. Forest plots were made on the basis of random-effects models. For the analysis, a total of 272 subjects representing eight studies were eligible.
The pooled rates of gastroparesis cardinal symptom index (GCSI) at preprocedure,1–3 months, 6 months, and 12 months are depicted in the following table:
The pooled results of 4-h gastric emptying scintigraphy (GES) pre- and post-G-POEM is shown in the following table:
Moreover, the pooled clinical response rate was found to be 84%. The GES normal and improvement rate was also examined, as depicted in the following table:
The pooled adverse events rate was found to be 12%. Thus, G-POEM is a beneficial
approach to manage patients having gastroparesis. However, future studies are
required to determine the most beneficial subgroup of patients due to G-POEM and
specify
its indications.
